Help w/bolt missing from engine (pics)

well... I finally thought about looking under the hood... guess what? that freakin' bolt is missing... of course!
What's the deal with this? Did they not tighten it enough? Or is it really in need of a lock washer...
After replacement, has anyone had it fall out again? seems like it might be a good place to put a little lock-tite in to keep it there. Or maybe a star lock washer?
I have my appointement set up for tomorrow at 8:45am. When I told the guy over the phone about it he said that he had never heard of that happening before. I'm thinking that for grins I will print out this thread to show him that it is pretty common.

For those that want to do this high-tech heat shield bolt replacement themsleves, get a hex-head bolt with a 6mm thread, about 12 mm or 1/2" or so long, and an appropriate lockwasher to improve the odds it will stay there. Be careful to not overtorque it and snap it...

Once replaced it will come back off and get a lock washer though.

Well a trip to the dealer, 10 mins (after I talked to the tech, the manager wanted me to leave it) and I have a new bolt...
The tech had also heard that this was becoming a common problem with these. Not one that is hard to fix, just an annoyance.
