Higher octane just has a higher resistance to pre-ignition or "knock". It's better for a high compression or forced induction engine. I doubt there are any MPG increases based on it, and even if there were, the price would cancel out any savings.

Kind of like it's cheaper to drive a 78 Monte Carlo than a new Prius. Sure you pay a lot at the pump, but how many fill ups is $22,000?
