What is required to convert rear drums to disc?

What is required to convert rear drums to disc?

This is for my wife's 03 LX. Will I need the master cyl off a disc car? What about the proportioning valve?

I am ok with making hard lines if I need to.

Can I just pull the entire rear hub/backing plate/caliper/rotor assembly off and bolt it on?
